You can't train health care workers "on the job". Many positions require months or years of classroom & clinical experience followed by taking a certification or state board examinations. Even changing someones diaper and emptying their bedpans requires an "CNA" certification. And the pay sucks. But benefits are usually available and there is always room to grow. Many go on to nursing degrees.
